- One hardcover booklet entitled "Lake Carriers' Association Certificate of Membership in Welfare Plan and Discharges." The pocket-size booklet has a dark blue cloth cover with white title text. Printed text on the inside front cover indicates that Arthur Page, an Able Seaman (Fireman), was enrolled in the benefit plan. The membership certificate "No. 101107" was issued at Detroit on September 6, 1922. The booklet contains 14 pages of blank lines that were to be filled in by ship officers to indicate the name of the vessel on which the seaman had served, rating, and dates of service.
